Information For Families

The Forget Me Not Children's Hospice will begin to deliver care in the hospice from December 2012 in addition to the Hospice at Home service that has been operating for more than a year.

 

Hospice at Home care

The hospice care team currently provide home based care for children and young people between the ages of birth and 19 years old who have a life limiting or life threatening condition.

We offer care in your home provided by our team of nurses, nursery nurses, play specialists, social workers, physiotherapists, health care workers and complimentary therapists. We work closely with the other professionals that are involved in your child’s care to ensure the services we offer are seamless and enabling us to provide the best possible care.

We will provide each family with a number of respite hours per week, based on individual needs, which families can use to meet their needs.

In addition to planned respite care, we also provide:
· Emergency respite
· Symptom control care
· End of life care
· Bereavement support

When a child reaches the end of life stage, our care team will endeavor to support the family on a daily basis with 24 hour on call support at home.

 

The Forget Me Not Children's Hospice

This will take place in the lovely surroundings of the recently completed Forget Me Not Children's Hospice; Russell House in Bradley near Huddersfield. The large, cosy house with four bedrooms, an impressive array of play and teenage rooms and a wonderful, brightly coloured communal space at the heart of the building will welcome families from early 2012
