Having a criminal record will not necessarily bar you from volunteering with us. Please contact the Volunteer Team if you wish to discuss this further.
As an employer, we have a responsibility to prevent illegal working and to ensure you are legally entitled to work in the UK. Are there any restrictions to your residence in the UK which might affect your right to take up employment with us? If you are offered a position with the Hospice you will be required to provide original documentary evidence of your eligibility to work in the UK.

This reference request will only be used for the purpose of fulfilling this application. Upon submission of this form, we will contact your referees and ask them for the following information:

- Information on your character, including reliability and capability to carry out the volunteer role

Health & safety agreement
All volunteers must follow the arrangements described in the FMNCH Health and Safety Manual and safe systems of working.
We would remind you that volunteers have duties under The Health and Safety Work Act to:
- Take reasonable care of your own health and safety and that of anyone affected by what you do.
- Be aware of how your activities may affect other people.
- Co-operate with FMNCH rules and procedures that are in place for your health and safety.
- Do not misuse any equipment provided for Health and Safety reasons.

I understand that whilst volunteering at Forget Me Not Children's Hospice I may see information about children and their families, patient care, fundraising, financial information, staff, volunteers, sponsors and suppliers. Any information I receive is given in the strictest confidence and I will not disclose it to anyone outside of Forget Me Not Children's Hospice, both during my time volunteering and after. Forget Me Not Children's Hospice will deem any breach of confidentiality as a serious offence and appropriate action will be taken.
