Have you ever been convicted of a criminal offence?
(Declaration subject to the Rehabilitation of Offenders Act 1974) Any offer of a volunteer post is subject to satisfactory references and a check from the Disclosure and Barring Service (DBS) (if appropriate to the role). Having a criminal record will not necessarily bar you from volunteering with us. Please contact the Volunteer Team if you wish to discuss this further. 
Do you have the right to volunteer in the UK? If you are not a British citizen, you may need to check your visa
UK citizens and citizens of EEA countries are free to volunteer. If you are a visa holder from outside the EEA you must contact UK Visas and Immigration to find out if your visa allows you to volunteer. We will ask to see your visa if this applies.

I understand that whilst volunteering at Forget Me Not Children's Hospice I may see information about children and their families, patient care, fundraising, financial information, staff, volunteers, sponsors and suppliers.
